The Patient • 80 year old white male • Patient presented with a purple lesion without symptoms with unknown duration • Pt is currently taking Isosorbide Mononitrate, Multivitamin, Niaspan, Nitroglycerin, Octuvite, Fish Oils, Omega-3 Fatty Acid, Omeprazole, Restasis, and Simvastatin • Pt has never been told about the lesion.

  4. The Lesion • The lesion is a solitary purple lesion located on the left posterior border of the lower lip on the oral mucosa. • The lesion is a small purple papule raised approximately 1mm and 3mm in diameter, flatulent, with well-defined borders, with apparent blood vessel involvement.

  5. Differential Diagnosis • Varix • Mucoceole • Melanoma • Hematoma

  6. Patient Dialogue • When did you notice the lesion? • How long have you had the lesion? • Does the lesion ever change in size or location? • Do you have any symptoms with this lesion? • Have you ever been diagnosed with a systemic disease? • Have you ever noticed any other skin lesions?

  7. Provisional Diagnosis/Treatment • Varix • No treatment is needed for this lesion. Lesion might resolve on its own but usually will remain. • Incisional biopsy is indicated in any significant changes in symptoms and size for malignant tendencies.
