Background • Syphilis is epidemic among gay and other men who have sex with men (MSM) in California • 85% of primary and secondary syphilis cases were among MSM (California, 2001 – 2003) • Traditional syphilis control measures focus on notification, testing, and treatment of locatable sex partners

Methods • Operational • Disease intervention specialists (DIS) interview infectious syphilis cases for patient/partner management and surveillance purposes • DIS attempt to contact sex partners • “Non-locatable” sex partners include: • “anonymous” partners • partners known but not disclosed to health department staff
Venue information • “Venues” for meeting partners include: • The Internet is a virtual venue; includes chat rooms, online personal ads, and sex parties posted online
Results • 76% of P&S MSM cases reported any venues from 2001 – 2003 • Top 4 • -Bars/clubs (31%) • INTERNET(28%) • Bathhouses (20%) • Sex clubs (9%) • 67% of P&S MSM cases reported at least one of the Top 4 • 1.3% (5 / 375) heterosexual syphilis cases reported the Internet 3/04 Provisional Data - CA DHS STD Control Branch

Conclusions • Among California MSM P&S syphilis cases: • The Internet has become a commonly reported venue • Cases using only the Internet have substantially increased • >80% of partners among cases reporting the Internet were “non-locatable” to health department • Because a significant number of partners are “non- locatable”, traditional syphilis control measures alone are not an effective control strategy
Future research: • HIV sero-sorting and the Internet • Percentage of MSM without syphilis utilizing the Internet • Characteristics of MSM using the INTERNET only • Proportion of “non-locatable” partners truly anonymous versus those who could be located through other strategies Recommendation • Comprehensive syphilis control strategies should incorporate appropriate Internet-based interventions
