Commissioning in E&N HertfordshireA history of strong clinical leadership with good health outcomes and a good financial track record 2007 • Formerly 4 PCTs in East & North Herts • Became 1 PCT but retained local focus through 6 Practice Based Commissioning Localities • Delivering Quality Health Care for Hertfordshire – Clinically led whole system reconfiguration – for East & North Herts :- • Lister to develop as acute site with development of New QEII as new commissioner led facility; Care Closer to Home; Urgent Care reconfiguration • Period of consolidating locally led clinical commissioning expertise • Both Herts PCTs joined together to form NHS Hertfordshire • Surgicentre and New Maternity Wing open on Lister site; • Business cases for Emergency Care Centre approved by PCT and supported by CCG • New QEII FBC designed and led by CCG • New Wards & Theatres FBC supported by CCG • Planning for our priorities: • Facilitating self management and promoting personal health planning for frail elderly and patients with LTC. • Developing culture of proactive management of frail elderly and patients with LTC. • Working to improve outcome for frail elderly, patients with LTC and End of Life Care. • Achieving Financial balance 2007 2010 2011 2012 2013

Our aim is to work with patients, managers and clinical colleagues from all sectors to commission the best possible healthcare for our patients within the available resources’” Needs assessment mapped to communities and groups Priorities established within the JSNA Contracts, QIPP and other commissioning levers used to maximise performance and outcomes within a cost envelope Integrated plan describes commissioning intentions Practice monthly performance reports to ensure delivery Operating Framework and CCG work plan describes detail of plan and finances Where appropriate, the CCG works with neighbouring CCGs or other partner organisations
