Unified Assessment Implementation Strategy for Conwy and Denbighshire NHS trust
Why are we introducing Unified Assessment? • The Welsh Assembly Government have issued directions which requires each local authority and their health Partners to introduce Unified Assessment for older people by April 2005 and all adult services by April 2006
What is the time scale for trust implementation • It is anticipated that nearly all areas within the trust implement some Part of the process by July 1st 2005
Which parts of the process must we implement • Introduction of the Summary record • Introduction of the Users Perspective • Introduction of the trigger sheet • Introduction of collaborative care plans
Impact within health • Disadvantages Although for those patients requiring Unified assessment there is the added responsibility for producing a summary record at the end of an acute episode • Advantages Any patients who have been assessed prior to admission will arrive with a summary of the needs and services they are getting in the community this should produce great time savings for staff.
Impact on Health • Named Nurses on the wards will become care coordinators for the period of time that the patient is in their care. • Allied professionals will need to ensure that they provide the named nurse with any treatment plans/care plans which should be placed in the Patients notes.
Impact within Health • The medical records will be kept in the same way that they are kept now. • Although copies of the summary records and discharge plans leave with the Patient, the originals are still kept within the trust. • Nurses continue to assess patients using the specialist nursing assessment • Improvements will need to be made within the care planning process across the whole hospital and to ensure that care plans and core plans do relate to the Nursing assessment • Identification of needs is not the responsibility of the discharge liaison nurse
What are your responsibilities within the implementation process • To be clear that you understand what is being asked of you • To actively work towards developing innovative solutions in order to make implementation happen • To be responsible for identifying the key inhibitors to implementation
Unified Assessment is not an option • This Guidance issued under section 7(1) of the local authority social services act 1970. • They will also provide the basis for use of new joint working flexibilities incorporated in the Health act 1999.
Programme for the next few months • Tasks which need to be completed • Implementation plan • Cathy to introduce the project plan and guidance
What needs to be done • Each ward needs to set up it’s own implementation group • Consider who should be involved within your team implementation group • Agree how often the group will need to meet • Who will represent the team at project task and finish groups
What is being asked of you? Nominate a lead officer • The lead officer will be responsible for reporting progress to the project team • They will need to highlight any issues which are making implementation impossible. • Select a date for implementation
Nominate a lead training officer • They will report to the Training task and finish group • Ensure that the training needs of the staff on the wards are identified • Arrange the training and support of staff • Report any issues to the lead officer
Clear Vision of role within the UA model • Specialist assessment • Care coordination role • Discharge planning

What will the group have to make decisions • Consider what administrative changes need to be made • Files • Photocopying • How will they get enough assessment forms available in the team • Who will keep supplies available
Piloting • Decide how you are going to implement Unified assessment • Phased approach, • trigger sheet • Users perspective • Summary record. • Consider how you will evaluate the impact of UA on the team
Training • Decide on the training approach • training key people and cascading down or training teams in team meetings • Liaise with Project manager • Identify future training needs • CHC information sharing protocol
IT • Identify the number of computers you will require for future processes • Identify a lead practitioner who can feed into the informing health care project
