  4. RADIOTHERAPIE EN BORSTRECONSTRUCTIE

  5. LYMFOEDEEM EN BORSTKANKER • Circa een kwart van de patiënten die een behandeling voor borstkanker hebben ondergaan • Incidentie - 24% tot 49% namastectomie - 4% tot 28% nalumpectomie - 5% tot 7% na sentinel node bioptie • Groterrisicobijokselklierdissectie en radiatie

  6. PARADOXEN • Niet alle borstkanker patiënten hebben lymfoedeem • Niet alle patiënten met OKD hebben lymfoedeem • Niet alle patiënten met radiotherapie van oksel hebben lymfoedeem • Ook jaren na behandeling nog mogelijk

  7. AARD VAN HET PROBLEEM • Onderbrekinglymfe-transport systeem • Accumulatieeiwitrijklymfevocht in interstitiëleruimte • Vroege manifestatie als soepel, pitting oedeem • Later progressienaarnonpittingoedeem en irreversibelevergroting van extremiteiten door ontstekingsreactie en vetdepositie • Vast subcutaan weefsel, progressief tot fibrosevorming

  8. GEASSOCIEERDE PROBLEMEN • Toegenomen gevoeligheid voor infectie in aangedane extremiteit • Cellulitis bij minimaal trauma • Episode van infectie geeft verdere schade aan lymfebanen (vicieuze cirkel) • Soms levenslange antibiotische profylaxe

  10. Lymfangitis • Ontstekinglymfekanalenalsresultaat van eeninfectieaan het distaleuiteinden (paronychia, insectenbeet, intradigitale infectie )

  12. conservatief • Symptomatischeverbetering van lymfoedeem • Mogelijkremmen van progressie • Zwachtelen (meerderelagen, kort-elastisch) van distaalnaarproximaalvoorlymfestroomnaarproximaal

  13. Conservatief • Op maat gemaakte drukkleding voor behoud van verminderd volume extremiteit

  14. Nadelen conservatief • Zonderpassendedrukkledingslechtskortetermijnresultaat • Tijdrovend en arbeidsintensief • Gespecialiseerdetherapeuten • Toewijding van patient en socialenetwerk • Zelf aanbrengen van zwachtel moeilijk • Geenverandering in omvang door vetdepositie en fibrose

Pathofysiologie ALNT • Vrijprepareren van littekenweefsel, welke mogelijk de lymfatische flow blokkeert • Gezond, gevasculariseerdweefseloverbrugtmogelijklymfatische wegen door litteken • Promotie van lymfangiogenese door vasculairendothelialegroei factor C (VEGF-C) in gezonde lymfeklieren • Gezonde lymfeklieren hebben mogelijk voordeel bij een lymfoedemateuzeextremiteit die eerderblootgesteld is aaninfectie
