Supervisor evaluation Important !! Upon completion of your practical training, need to get your supervisor to : 1. fill in the evaluation & attendance forms. 2. sign and rubber stamp the forms 3. put the forms in an envelope 4. give the envelope to you (for you to submit by hand to your academic supervisor, together with your practical report)
PRACTICAL REPORT • Formal report format ( 3 sections) • Preliminary, Main, Supplementary sections. • Focus on activities / job done at workplace • Hard cover report – Dark blue. • Dead line – (24 April 2014 -Thursday) 3 weeks upon completion of the practical training
PRACTICAL REPORT Focus on activities / job done at workplace • Based on what is written in the log book • Supported by evidence e.g. • any written draft that you’d prepared, • minutes of meeting that you had attended /prepared, • your photos attending a function /doing activities, etc • Evidence of job done is to be put in the Appendix. • Appendices should be named accordingly • E.g. Appendix 1a, Appendix 1b, Appendix 2a etc.

PRACTICAL REPORT – Main section i. Introduction - About of practical training program , - MARA’s vision etc. ii. About the workplace/organization. iii. Practical training activities -work done at the organization – Based on what is written in the log book iv. Conclusion Reflection (what you have learnt there) Recommendation (for KPM/MARA, the organization, future trainee ).

PRACTICAL REPORT – Headings of activities • Classify according to the nature of work. • 1. Human Resource (on employee relations) • Administrative work (attending meeting etc) • Writing tasks (formal letters, minutes of meeting, etc) • Clerical job ( key-in data, filing etc) • 2. Public Relation (company image building) • Event management ( help organizing event etc) • Customer service (front desk receptionist etc) • 3. Marketing /Sales (product/ service) • Promotional activities • 4. Computing Task
Example of Practical activity entry • 1. Human Resource • 1.1 Clerical works is part of human resource and at MOMSB, it is done by everyone in the organization. The clerical works here include the preparations of documents, tenders, quotations and other related document or tying work. The clerical jobs which I had done in MOMSB were creating back the tender for University Malaysia Terengganu. The task was to retype the whole tender paper in soft copy looking exactly like the original configuration as the soft copy will be used back as a record for the company.(Please refer to appendix 1a)

LOG BOOK / JOURNAL ENTRY • Note down your daily job. • to be endorsed daily or weekly by the supervisor : • approving that he has assigned you the job • that your have done the job given. • The ‘remarks’ section is for your reflection of the job done. • Example of how log book is written.
LOG BOOK / JOURNAL ENTRY LOG BOOK (10%) • Daily details on work /tasks done - 25 % • Brief reflection on the tasks done - 15 % • Neatness - 5 % • Supervisor’s verification - 5 % (on daily or weekly basis) Total marks 50 %
Academic supervisor (AS) Need to see your supervisor personally : i. get to know him/her better ii. ask for contact number & Email address iii. to inform your AS who your workplace supervisor is & his /her contact number (first week of your practical training)
Academic supervisor ‘s job 1. Liaise with your workplace supervisor to know about your performance, progress, attendance etc. 2. Advise you in any academic problem you encounter in the workplace. 3. Mark your log book and practical report.
‘Dos’ in the workplace • Always be polite, humble and friendly. • Do respect and smile at everyone. • Build a good relationship with the staff –esp. your supervisor as he/she will determine your marks there. • Always accept any job with a smile, no matter how ridiculous you thought the jobs are.

Always ask for jobs to do. • Show that you are interested to learn and that you are a very proactive person. • if you don’t have any jobs / tasks to do, you will not have anything to write in your practical report which eventually can lead to your failure. 6. Ask the supervisor if you don’t know how to do the job. Nothing to be shy of – remember you are there to learn.
