Health Assessments for Children in Care Initial Health Assessments (Within 5 workings of a child becoming LAC) & Adoption Medicals All Review Health Assessments Send to Virtual School Kent Health VSK_Health@kent.gov.uk For all Kent Children in Care placed in Kent, Medway & Out of Area Send to Central Inbox to Children in Care Coordinator ECS-TR.VSKLAC@nhs.net For all Kent Children in Care placed in Kent, Medway & Out of Area ADOPTION REVIEWS INITIALS Initial Health Assessments Paperwork to be included: 1.IHA-C or IHA-YP BAAF Form – Page 1 & 2 must be fully completed 2. Placement information record with consent that must be signed 3. Any other relevant information i.e. ICO – copy to be attached. 4. Standard request letter All Review Adoption Medicals Plan changed from Initial Health Assessment to Initial Adoption Health Assessments where child had an Initial LAC Health Assessment within 3 months Send to Central Inbox to Children in Care Coordinator ECS-TR.VSKLAC@nhs.net For all Kent Children in Care placed in Kent, Medway & Out of Area CICCO to liaise with Medical Advisors so that a clinical decision will be made as to whether they need to continue to see the child or the review health assessments can be undertaken by the LAC nurse. However, if the Review Adoption is carried out by a Nurse, subsequent requests still need to be requested through the above mentioned e mail box. ECS-TR.VSKLAC@nhs.netso that the case will be reassessed and a decision made as to who will undertake the next review Adoption Health Assessments Paperwork to be included: 1. Completed IHA-C or IHA-YP BAAF Form 2. Completed RHA if available. 3. Completed Medical Consent form as per status 4. Completed MB 5. Standard request letter 6. Completed form PH. 7. Any available expert reports relevant to the Child. 8.Form C – enclose copy of the Report. If the child has had a recent IHA (approx 3 months) and the plan has changed to Adoption, CICCO to liaise for decision to be made as to whether child needs to be seen or Adoption medical advice can be offered based on last Health Assessment
