Purpose of the CHART • Convene stakeholders from multiple sectors that are committed to promoting community health • Assess& monitor status of health for your local community(i.e., Physical Activity/Nutrition/Tobacco-PANT) • Develop a CAP (Community Action Plan) to promote physical activity, good nutrition, and tobacco cessation • Implement strategies which achieve increased physical activity and good nutrition • Use the CHANGE tool to evaluate and gather data on the impact of your ACHIEVE efforts • Develop and implement strategies to sustain the movement
