E N D
Purpose • Through a team approach, identify those individuals (residents) who have demonstrated physical and/or cognitive changes: then communicate and coordinate available options to those residents and their families to support their highest quality of independence.
Confidentiality • The Committee follows the Health Insurance Portability and Accountability Act (HIPAA) guidelines as to the use and/or disclose your health information. Generally it is for purposes of treatment, payment, or health care operations.

Conditions of Tenancy • PURPOSE OF THIS DOCUMENT: • To provide guidance to the primary care physician and staff in determining eligibility of prospective and current residents for tenancy. • To provide guidance to the staff when a resident no longer meets eligibility requirements. • To provide information to individuals and their families as to eligibility.
Health Maintenance Watch List • What type of occurrence would cause someone to be placed on a watch list • Recent surgery • Recent hospitalization • Family concerns • Repeated falls • Increased confusion • Medication errors • Not managing Activities of Daily Living (ADL)
Tools for Physical and Mental • Functional Assessment – measures the ability of the resident to care for themselves and/or the amount of care needed. • Mini-Mental State Examination – measures the cognitive ability of the resident.

Third Party Service Providers • Policy – Employment of outside individuals may not begin until all pre-hiring requirements are met. • Purpose – To ensure the safety of residents, family members, and employees.

Motorized Ambulatory Aides • Montereau supports Resident use of Motorized Ambulatory Aides as an assisted device to meet the Independent Living “Conditions of Tenancy”. • Residents with Motorized Aides are responsible for damage to Montereau or injury to others.
