What is Dental Charting for? • Dental Charting is a task that Dental Assistants will do for the dentist on a daily basis. You will need to become familiar with surfaces of a tooth, existing conditions, and restorations.
Terminology • Maxillary – Upper Teeth • Mandibular – Lower Teeth • Anterior – Front Teeth • Posterior – Back Teeth • Midline – Imaginary line down the very center
Surfaces • Mesial – closest to Midline • Distal – farthest from Midline • Lingual – closest to Tongue • Facial – closest to Lips (Anterior) • Buccal – closest to Cheeks (Posterior) • Incisal – Biting Edge of Tooth (Anterior) • Occlusal – Chewing Surface (Posterior)
The adult mouth normally has 32 teeth. • Extraction of Wisdom Teeth is normal.
Teeth are numbered from 1 to 32. • On the upper arch, from left to right (dentist view) going from 1 to 16. • Then on lower arch, from right to left, going 17 to 32.
The “Midline” is an imaginary line that goes down the center, as illustrated by the turquoise line. • The “Lingual” would be the surface closest to the tongue, as illustrated by the green line.

Distal • Surface Farthest from the Midline • on All Teeth
Lingual • Surface Closest to the Tongue • on All Teeth

impacted • Permanent Tooth has not Erupted • Circle around the outside in red • write IMP ATTENTION WORTHY! MARK IN RED!
Amalgam • Silver-colored Mercury Filling • Fill-in indicated surface
Composite • Tooth-colored Light-Cured Filling • Circle around indicated surface

root canal • Nerve removed from tooth and replaced with rubber material called Gutta Percha • 3 lines away from lingual • Blue or Red
abscess • Tooth Infection • a red pimple and 2 lines ATTENTION WORTHY! MARK IN RED!
apicoectomy • Root Slice • line thru the root
bone absorption • v-shape
partial denture • M over missing tooth • boxes over anchor teeth • line on the bottom
3 unit fixed bridge • M over missing tooth • Circle Abutment Teeth • 2 lines in between
Where are the Midline and Lingual lines? • Midline • vertical line that goes down the center • between teeth #8 & #9 • also between teeth #25 & #24 • Lingual • horizontal line which represents the surface closest to the tongue • between Maxillary & Mandibular arches
What is the Mesial? • Surface Closest to the Midline • on All Teeth
What is the Distal? • Surface Farthest from the Midline • on All Teeth
What is the Lingual? • Surface Closest to the Tongue • on All Teeth
What is the Facial? • Surface Closest to Lips • on Anterior teeth
What is the Buccal? • Surface Closest to Cheeks • on Posterior teeth
What is the Incisal? • Biting Edge • of Anterior Teeth
What is the Occlusal? • Chewing Surface • of Posterior Teeth
Which color pencil would you use to indicate work that needs to be done? Red
What color pencil would you normally use to record Existing Conditions? Blue
any attention-worthy issue must be in Red, even Pre-Existing! Red But what if it’s a pre-existing issue that’s in particular need of special attention… what color pencil would you use then?
If a Tooth is Missing, how do you write it? • mark with a blue M
How do you mark a tooth that needs to be Extracted? • Mark with a red X
How do you mark down an Impacted tooth? • Circle around the outside in red • and write IMP ATTENTION WORTHY! MARK IN RED!
How do you mark Amalgam filling? • Fill-in indicated surface • Blue for existing • Red for to-be-done
