Practice Update On Line Services: - Target 25% Prescriptions, Appointments and Summary Care Record 1491/9330 – 15.6% Register at Reception with Identification Electronic Prescriptions: Order and collect from a nominated Pharmacy, you choose. Repeat Dispensing: Patients who are stable on routine medication can be included in this scheme for multiple prescriptions to available direct from their chemist. Ask the nurse or doctor at your next review.
Repeat Prescription Requests January 2017: The practice will no longer accept orders over the telephone unless exceptional circumstances are approved. Online – Computer or Mobile Phone App Reception drop off Post We encourage all patients to register for on line access.
Services Update • 2016 Flu Campaign: Vaccinated: 2068 Community Clinics held at Townend Avenue (8) and St Clements Court (6) 14 Attended – Propose not held in 2017 • Extended hours ending 31.03.2017 Tuesday early morning and Saturday mornings. The CCG are withdrawing this contract and are looking to develop the urgent care and extended hours centrally. • Sensory Impairment Training Sensory impairment champions, selected staff will complete training to enable us to be “sensory impairment friendly” .
Care Navigation Ringing your GP practice for an appointment to see the GP can sometimes seem a ‘lottery’. Care navigators will guide patients to health care options. Trained frontline staff will ask what the problem is. This isn’t to be nosy but it’s so they can help you get to the right health professional for your needs. They have a multitude of information and knowledge about services outside of the GP practice that can help too if you need that. Receptionists are receiving training to enhance their ability to connect patients directly with the most appropriate source of help.

National Cancer Audit The National Cancer Diagnosis Audit (NCDA) will provide new insights that will inform service improvements that in turn help to diagnose cancers earlier and improve cancer outcomes. Dr Roche will be leading our audit of 55 patient cases and access their patients' records and input their data onto the audit portal.
