  13. Form DR23, Degree Request, as needed Form 450, District Deputy's Degree Exemplification Report, after each Degree (Complete a separate from for each Degree. Do not combine Formation and Knighthood Degrees on one form.) Admission, Formation & Knighthood Degree Forms

  14. Degree dates and information must be posted on the “State Website Degree Schedule” Recommended: Notify Council FS and District Deputy of candidates that the candidates received Degree(s). Members' FS (not the host council) is responsible for notifying Supreme. Admission, Formation & Knighthood Degree Forms
