USAC Rural Health Care Pilot Program • September 26, 2006 • FCC Announced establishment of pilot program for public and non-profit health care providers to build state and region-wide broadband networks dedicated to the provision of health care services • Pilot program will fund up to 85% of the cost

Indiana Telehealth Network (cont’d) • Phase 1 • Construct Fiber-Optic Connection • Participant hospital’s Network Interface Device (NID) to nearest Point of Presence (POP) • Bid monthly recurring Internet Access • 10 mbps, 50 mbps, 100 mbps, or 1 gbps fiber-optic connection* • Phase 2 • Connect participant hospitals to common meet point(s) • Internet Access at the meet point(s) • Bid monthly recurring Internet Access • 10 mbps, 50 mbps, 100 mbps, or 1 gbps fiber-optic connection* • Funding • 85% of construction costs to be paid from FCC Pilot Program funds • Remaining 15% to be split between participants, state of Indiana, Indiana Office of Community and Rural Affairs (OCRA), stimulus funding, and/or other funding • Expect 7.5% from individual participants • Expect 7.5% from state of Indiana/Stimulus/Grant funding • Expected outcomes • CAHs will be anchor tenants in their communities • Telecommunications providers will be tasked with continued residential and business build out * Note: A T1=1.5 megabits, a T3=45 megabits

Benefits of Telehealth • Electronic transfer of PACs images to remote specialist for timely readings • Increased community effectiveness • Increased opportunities for staff recruitment, training, and retention • Increased patient outreach programs • Keep patients local • Less drive miles for parents when caring for children • Less missed work for parents and patients
