Focus on Home Telehealth • Context • Care Provider is the Nurse primarily • The Patient is infirm and confined to home • Patients are not in urban areas • Episodes of care last weeks to months • Prospective Payment System is in place • Patients costs reimbursed by Medicare (>75%) • Low affordability

Sub-Focus on CHF/COPD • Assumptions • In the first instance, ‘telemedicate’ current procedures • Certain vital signs are key indicators • Voice is paramount, Still image is good, Video is a distraction if jerky • Low complexity on patient side is important for acceptance • Need to preserve Visit notes and follow-up

  4. Criteria of Evaluation • Technical • Primarily on a Testbed without patients • Clinical • Primarily on a Testbed with simulated patients • Usability • We look forward to the possibility of a real-life setting

  8. Simulated Testbed • Medical Instruments may be simulated • Communication interfaces can be simulated • Patient databases can be simulated • Even PDAs have simulations Hence a Look and Feel can be put together

  9. Going Beyond Home Care • Bringing the Physician into the loop • Making possible procedures for which a patient is now transported to a clinic (e.g. EKG) • Many patients at one site, e.g., Elder Home • Service to monitor Chronic Care patients Medical Kiosks of High Configurability
