Completing the Registration Form • Refer to the checklist on the back of the form • Fill out Registration Form completely. All boxes should be filled. List alternates in priority order in the boxes on the lower half of the form. • Requires a parent / guardian signature. • Forms for Sunset students are due Friday, February 10th. Requests will be entered in computer labs at Sunset. • Forms for students from other middle schools are due to NHS no later than March 1st
