  1. 2014-2015 At Risk After School Supper Program Site Monitoring Training for Food Services Staff Provided by the LAUSD Food Services Division 08.01.2014

  2. Overview This training will provide attendees with the knowledge to understand the Supper program processes and requirements. This training will address the following areas: • What is After School Supper • Required documentation • Role of the Food Services staff • Monitoring requirements • Responsibilities of the Afterschool Program staff

  3. Overview • Why are site monitoring conducted? • What are the monitoring requirements? • What are the corrective actions requirements? • What is the required documentation?

  4. Goal • Our goal is to conduct the three (3) required site monitoring on a timely manner to ensure the program is in compliance with the USDA and the CDE rules and regulations • Provide necessary corrective actions in accordance with the site monitoring findings • Conduct follow-up training as needed

  5. Annually Monitoring Requirements The Food Services Manager or designee is required to conduct: • 3 mandatory site monitoring to ensure the program is following proper procedures • 1 site monitoring may an announced • 2 site monitoring’s must be unannounced • If corrective actions are needed additional follow up monitoring's will be conducted Coordinate with the AFSS and communicate any concerns or issues discovered during the monitoring

Procedures for Corrective Actions • Corrective actions • Any corrective actions require • Follow up must be completed within 60 calendar days • Discuss the monitoring’s finding and any necessary corrective actions with the afterschool program coordinator. • Provide a copy of the monitoring to the after school program coordinator. • All site monitoring’s and corrective action documentation must be filed and keep for 3 years.

  7. Monitoring Timeline • The first monitoring 4 weeks after the start of program • Completed before September 10, 2014 • No more than six months may lapse between reviews • The second monitoring must be conducted prior to winter break • Completed before December 19, 2014 • The third monitoring must be conducted prior to spring break • Completed before March 27, 2015

  8. Submission of Monitoring Reports Send a copy of the monitoring to the Beaudry Building - Food Services, 28th Floor. Attention: Food Service Staff Aide (per each ESC) • North ESC – Lisa Hess • South ESC – Olukemi (Felicia) Coker • East ESC – Abyz Dayrit • West ESC – Edna Gabriel • Give a copy to the program coordinator • File and keep for 3 years

How to Complete the Heading • Fill in the information in the heading section: • Facility/site name • Date of monitoring • Time you arrived to site • Time you left site after monitoring • Circle whether this was an announced or unannounced visit • Which meal was observed • The time the meal was served • Total number of children served • The attendance for date observed • The meal count for date observed • Avoid conducting the monitoring on the same day of the week, such as every Tuesday or Friday.

Summary of Findings and Recommended Corrective Actions 09/04/13 Rowan El Supper 5 Meals were observed to be taken offsite by students- Recommended corrective action reinforce with afterschool program coordinators and staff that meals must be consumed on site. Meals were not recorded on the reimbursable meals worksheet at the time of meal service. Meals were marked after all students received a meal-Recommended corrective action reinforce with afterschool program coordinators and staff that meals must be recorded after the child has taken all menu items.

Responsible Individuals The following individuals are responsible for conducting and verifying the Supper site monitoring's were conducted according to the timeline: • Food Services Manager or designee is responsible for conducting the required site monitoring’s • Food Services Manager or designee is responsible for verifying that all corrective actions have been taken • Food Services Manager or designee is responsible for scanning and emailing the site monitoring’s to their ESC staff aid • AFSS must review and verify that all sites have submitting that all site monitoring’s and corrective actions have been completed by the due dates • AFSS should ensure retraining has been provided if deemed necessary • SFSS must review and verify with AFSS that all site monitoring’s and corrective actions have been completed
