Sense of Urgency • A sense of urgency is the attitude and process of treating key or personal matters as if one's life depended on it. Vulnerable Populations: Eliminate Health Disparities Vulnerable populations are defined as those who may need adjusted service because of (including but not limited to): disability status, income (payor status), race, ethnicity, language, religion, sexual orientation, veteran status, or poor access to health care services.
Developing systems for addressing vulnerable populations • Collection and Use of Data • Benchmark gaps in care/Detection of variations in care delivery • Clinical Quality Indicators • Medical Error • Hospital Readmission • CMS Core Measures • Cultural competency/training components • Available language services • Family/Community interactions • Religious beliefs affecting healthcare • Diverse health beliefs • How do you measure the effectiveness of the training – the adequacy of the services provided? • Voice of the patient/diversity on patient advisory committees
