Complete information makes the process go faster • Use letterhead and include your license number • Provide contact email and phone number • Signed by individual with signature authority: RSO, CEO, or Senior Management Official with responsibility for the Radiation Safety Program • Provide all necessary supporting documents • See RHB website for guides and forms: http://www.cdph.ca.gov/pubsforms/forms/Pages/RHBLicensingForms.aspx
RHB Amendment Process • Mail doesn’t come to us directly (several days) • Amendment request is date stamped and logged in to data base • If request is faxed or emailed directly to a license reviewer, it still goes in the queue. No cuts! • 50-100 amendment requests typically in the queue at any one time • RHB HP Staff still furloughed 3 days a month • Due to back log, typically assigned 4-5 weeks after receipt to one of 7 license reviewers (HP)
RHB Amendment Process (cont.) • HP reviews amendment request and pulls license file • Quality submittal? • Follows guidelines to process changes • More information needed? • We send you a letter • 30 day response needed or abandon • Amends license and writes justification for amendment request • Submits file for peer review • Suggests changes, corrects errors
RHB Amendment Process (cont.) • License file returned from peer to HP • Reviews peer suggestions and corrections and amends license as necessary • Submits amended license for signature • May be returned for additional corrections or signed by Senior HP (Ira) • Sent to support unit for processing • Copied, filed, mailed out
And a few other things… • A docket number is specific to one amendment request. • Do not refer to it for subsequent amendments as your new request will get a new docket number. • Example: 100410-1556 • RHB will do our best to return phone calls within 2 business days • We are responsible for over 800 medical, academic and pharmacy licenses
Can’t find your license? • Request for copy must be from RSO or individual listed under condition 14 or on the attention line of the license • Request must be on letterhead and signed by authorized individual • Fax request to support unit: 916-322-0639
Plan Ahead • Yours is not the only facility that treats sick patients, cancer patients or patients needing a specific procedure. • A request to EXPEDITE your amendment because you did not plan accordingly causes delays for other licensees who planned responsibly.
