Overview • FEMA US&R WMD equipment cache • Scott respiratory equipment • Chemical Protective Clothing (CPC) • Hands-on practice WMD PPE
Objective Upon completion of this module, students will be able to select, don, and work in the appropriate WMD PPE and execute emergency procedures
Centers for Disease Control and Prevention Morbidity and Mortality Weekly Report September 2002 Centers for Disease Control and Prevention Morbidity and Mortality Weekly Report September 2002
MMWR Report September 2002 • World Trade Center Related Cough • 332 Firefighters • One Emergency Medical Service Worker Each had four or more weeks of medical leave
Ongoing Issues • 173 (52%) show partial improvement • Remain on medical leave or light duty • Some pending disability retirement
Additional Ongoing Issues • Respiratory illness after WTC exposure • 358 firefighters on medical leave or light duty • 5 EMS remain on medical leave or light duty • 500 firefighters (4%) of 11,336 responders may qualify for disability
MMWR Conclusions • Need for respiratory protection • Respiratory protection not NIOSH certified or fit tested • Training on respiratory protection • Discomfort • Heat stress
Respiratory Protection • Facepiece with Kevlar head piece • Facepiece accommodates and will do all three functions • APR • PAPR • SCBA
Protection Factors • APR = 50:1 • PAPR = 1,000:1 • SCBA = 10,000:1 1 is no respiratory protection in place
APR • Fits into Scott Full Facepiece • Adapter allows NATO cartridge • Multi-Purpose Cartridge (MPC) Plus • Meets/exceeds NIOSH
PAPR • Two - MPC Plus canisters • Slight positive pressure • 10 hour battery life • Batteries in cache are for deployment • Can be used off shelf without charging
SCBA • 60 minutes carbon cylinder • 30 units per TF
Half-Facepiece Respirator • Good particulate protection • MPC Plus • Easy to change cartridge DON’T FORGET EYE PROTECTION!
Communication System • Ten per TF • May encrypt
Hazards of Working in CPC • Physiological (heat stress) • Reduced mobility • Limited vision (poor peripheral vision) • Reduced dexterity • May promote claustrophobia
Damage to CPC • Mechanical—cuts, tears, punctures • Thermal—no fire protection • Chemical—no ideal suit to protect against all chemicals Overalls protect CPC Overalls protect CPC
Damage to CPC (cont) • Permeation, penetration, and degradation (TF may have to augment sizes with grant money)

Agent Specific PPE • Biological • Radiological • Nerve and Blister • Choking • Blood
Nerve Agent Antidote • Good for nerve agent only • Use only on symptomatic and contaminated patients • Self or Buddy aid Atropine 2-PAM chloride CANA (diazepam)
Injector • Sharp needle will penetrate CPC
Atropine • Induce or exacerbate Angina Pectoris • Increased heart rate • Up to three injections to counter nerve agent • Evacuate individual
2 PAM Chloride • Reverses respiratory depression and muscle paralysis • Inject after each atropine injection
CANA • Convulsant antidote for nerve agent • One CANA per individual • Should be used for buddy-aid only
Side Effects • Dangerous hypertension • Irregular heartbeats
Self Injection • Hold in hand • Do not place thumb on end • Atropine first, then 2 PAM Cl
Injection Site • Inject in large muscle of thigh • Area hand width below hip to hand width above knee • Hold in place 10 seconds
Alternate Location • Thin persons inject in buttocks • Avoid nerve and bone
CANA • Given as buddy-aid • Only one per individual
Misuse • Mishandling that may result in accidental self-injection • Second greatest danger is unnecessary self-injection • Do not use unless • You show signs and symptoms, • You know you are exposed, or • You are directed
Potassium Iodide • Protection only for thyroid from radioiodines • Daily dose good for 24 hours
Summary • Scott respirators • CPC • Mark 1 • Hands-on activity
