Joint Operations JO EHS Bulletin 2010 Dec Prepared & Published Monthly by EH&S, F&S Group Sixth Annual Contractor EHS Forum KOTC Management Team Visits JO Accident & Lessons Learned Sixth Annual Contractor EHS Forum was held on 27th October, 2010. Management representatives of various subsidiaries of Kuwait Petroleum Corporation, Saudi Arabian Chevron, KGOC and business partners of JO apart from JO personnel attended the form. On Monday October 4th, 2010, while a dump truck was returning after unloading sand/soil at the construction site (outside of SC-22),the front edge of the dump box hit 8” Overhead vent pipe line. The pipe collapsed, hit the cabin, causing damage to the cabin and windshield. Driver suffered minor injury to his head. Kuwait Oil Tanker Co. Management visited Joint Operations as part of continuing site safety visits by KPC subsidiaries. They were received and accompanied during site visit by EHS, F&S Manager Hamad S. Al-Ajmi, EHS Supdt. Sati Al-Otaibi and Sr. Safety Engr. Mohammed Abbas. • What Went Wrong • The tipper box was not in lowered position and the driver was not aware of this since the tipper position indicator was broken. • The driver was not trained and was not aware of the hazards i.e: height clearance on the route. • Height indicator was not available at pipeline location. Open & Closed Hazard Reports Lessons Learned • Regular inspection of all equipments and vehicles must be conducted and maintain all safety devices. • Lower all raised parts like tipper box, boom etc to travel position before starting travel. • All drivers and equipment operators must be trained in safe operation and hazard identification specific to their task. • Job Safety Analysis must be comprehensive and shall include all hazards and mitigation steps for the job. • Use assistance of a signalman always in the work area and whenever required during travel. • Post height clearance indicator where ever pipeline, cable etc. cross roads. Fire Watch Training Emergency Response Drill Joined EHS Fhaid S. Al-Marri Dr. Tolo Dudani Presentations and group discussions on various EHS issues and a hazard identification demonstration were the main attractions. Contractors displayed tools, equipments & work practices for forum participants to identify hazards. Awards were presented to contractors and individuals for achieving milestones and commendable safety performance. JO General Manager and Manager EHS, F&S, recognized the planning team worked for the Sixth Annual Contractor Safety Forum for its success. Fhaid S. Al-Marri (Safety Engineer) and Dr.Tolo Dudani (Industrial Hygienist) joined JO EHS division. They bring to JO their valuable experience from oil & petrochemical industries An emergency response drill was conducted in drilling rig-16 in Humma field. Drilling Division, Fire & Security, EHS div, medical clinic and KDC drilling rig crew participated in the drill. The drill was conducted and monitored by Emergency Management process advisor and SEACOR environmental services representatives Fire watch training was conducted from 19th to 21st October for the benefit of business partners. The training program in English and Hindi was conducted by Ranjit Dash, Safety Engineer and Hamad Boressli , Fire Engineer.
