  1. WHAT IS PALS (Practice Activity and Leisure Scheme) ? • Local Exercise Referral Scheme. • Partnership scheme between local health agencies, Kirklees Council Communities and Leisure Services and Kirklees Active Leisure Trust.

  2. WHAT IS THE AIM OF PALS? PALS encourages people to become more active more often with a view to improving health, well being and quality of life, particularly with inactive people at risk of developing certain health conditions or currently with health conditions.

  3. WHO IS PALS FOR? Essential Criteria • Age 16 years plus • Inactive • Motivated/Compliant • Resident in Kirklees and or a patient registered with one of the General Practices responsible to the Kirklees PCT

  4. WHO IS PALS FOR Cont.? Selected criteria • A – Low self esteem, mild anxiety and/or depression • B - At risk of/have Coronary Heart Disease • C - Hypertension • D - Asthma and other respiratory problems • E - Joint pain, back pain, arthritis or similar • F – At risk of/have diabetes • G - Stroke • H - BMI >25 i.e. weight control problem • I – Fallen/at risk of falling • J – Chronic/persistent pain

  5. HOW WILL IT WORK? Referral via Primary Health Care Team or other Health Care Specialist Introduction to the facility Introduction to activity concepts 10 Week personalised activity programme 15 week revised activity plan 20 week revised activity plan Moving on appointment with support and encouragement to remain active for life Active Lifestyle Consultation Active Lifestyle Review (1) Active Lifestyle Review (2) Moving on Review

WHAT WILL HAPPEN AT THE PALS APPOINTMENT? • Assessment of current activity level. • Information on the benefits of active lifestyles. • Help to set realistic, achievable activity goals. • Assessments available to measure fitness and health status e.g. resting heart rate. • Personalised activity plans to suit individual needs, including home based workouts.
