Background • Population of the country is estimated to be 35 million. • The HIV/AIDS prevalence is 9.6% • Estimated number of people living with HIV/AIDS virus is about 2 million. • The country is embarking to WHO’s 3by5 mission to provide ARV to about 220,000 patients by the year 2005.
Drugs to be used: First line drugs for adults are: • Stavudine/Lamivudine /Nevirapine (d4T/3TC/ NVP) • Stavudine/Lamivudine/Efavirenz (d4T/3TC/ EFV) • Zidovudine/Lamivudine/Efavirenz (AZT/3TC/EFV) • Zidovudine /Lamivudine /Nevirapine (AZT/3TC/NVP)
Continue…. • These drugs will be used depending on the different conditions the patients will present with. For patients with Hb less than 7gm/dl: Stavudine 40 mg/Lamivudine 150 mg/Nevirapine 200 mg twice daily (> 60 kg body weight). Stavudine 30 mg /Lamivudine 150 mg/Nevirapine 200 mg twice daily (< 60 kg body weight).
Continue… For TB patients the regimen will be: Stavudine 40 mg/Lamivudine 150 mg twice daily and Efavirenz 600 mg once daily at night (> 60 kg weight). Stavudine 30 mg /Lamivudine 150 mg twice daily and Efavirenz 600 mg once daily at night (< 60 kg weight).
Continue… For patients who have Nevirapine intolerance, the regimen will be: Zidovudine 300 mg/Lamivudine 150 mg twice daily and Efavirenz 600 mg once daily at night.
Continue… Patients with peripheral neuropathy who cannot use Stavudine, the regimen will be: Zidovudine 300 mg/Lamivudine 150 mg/Nevirapine 200 mg twice daily. There are also second line drugs which includes combination of two nucleoside reverse transriptase inhibitors with one protease inhibitors.
Control of these drugs TFDA: Tanzania Food and Drug Authority • Control the importation of all drugs in the country including ARV’s. • Developed the guideline for the distribution and storage of ARV’s in the country. • Laboratory for the quality control of pharmaceuticals entering the country.
