Transition Services “Transition services are a coordinated set of activities for a student with a disability that is designed to be within a results oriented process, that is focused on improving the academic and functional achievement of the student with a disability to facilitate the student's movement from school to post school activities”
Transition Services Include … • Postsecondary education, vocational education, integrated employment (including supported employment), • Continuing and adult education • Adult services, independent living, or community participation • “Based on the individual student's needs taking into account the student's strengths, preferences, and interests.”
Typical transition goals • Student will complete career inventories. • Person responsible: Parent, in conjunction with school district personnel. • Student will visit Community College and the Office of Disabilities. • Person responsible: Student and parent.
Typical transition goals • Student will be referred to www.collegeboard.com for information regarding post-secondary education and training options. • Person responsible: Parent. • Student will investigate possible job shadowing opportunities • Person responsible: Student and staff.
Typical transition goals • Student will participate in the ASVAB assessment to determine aptitude. • Person responsible: Student and school district personnel • Student will register with Goodwill Industries. • Person responsible: Community service personnel. • Student will complete Keys to Work program. • Person responsible: Student and parent.
Typical transition goals • Student will improve math skills for employment. • Student, in conjunction with school district personnel. • Student will be provided with information about his rights under the Americans with Disabilities Act. • Parent, in conjunction with school district personnel.
