Request for Call DescriptionUp To 2 Contractors Will Be Selected • This Job Order Contract (JOC) is for the maintenance and as-needed: • Restoration of established right-of-way landscaping • Irrigation system repair and restoration • Including backflow prevention systems repair and testing and remote control of irrigation controllers for water conservation • Within City of Phoenix and Arizona Department of Transportation (ADOT) right-of-way (city side of sound walls/fences). • Work may also include clear-cutting fire breaks in washes, the repair and/or installation of chain link & wrought iron fences and access control bollards.

SCOPE OF WORK • Repair and/or restoration of landscape irrigation systems including backflow prevention system testing, repair/replacement. • Clear-cut fire line breaks and other pruning/debris removal in washes with limited access and in accordance with Section 404 of the Clean Water Act. • Herbicide/pesticide applications in compliance with the State of Arizona Office of Pest Management, primarily to inhibit and/or eradicate existing weed growth. • Planting, pruning and removal of landscape shrubs, ground cover plants, vines, and trees, including palm trees. • Utilize certified arborist for tree pruning.
SCOPE OF WORK • Work to be performed in strict compliance with Arizona Department of Environmental Quality and Maricopa County air quality regulations. • Repair and/or install various types of fencing and traffic control bollards. • Perform various methods of graffiti removal including, but not limited to, painting and sand blasting. • Repair and/or replace concrete pedestrian pathways and miscellaneous concrete flat work. • Coordinate traffic control based on ADOT and City of Phoenix requirements. • Contractor must have a “Class A General Contractor’s License”
The JOC Will Be: • For a 36-month duration; or • $ 7,500,000 per JOC • Whichever occurs first • The JOC will include an option to renew, based on satisfactory performance
Evaluation Criteria • General Information (15 points) • Experience and Qualifications of the firm • (25 points) • Experience and qualifications of the firm. • (20 points) Approach to Performing the Required Services (30 points) • Overall Evaluation of the Firm and ITS ability to provide the Required Services. (10 points)
Submittal Requirements • Submit 9 copies of the SOQs • Maximum 10 pages • Submittal Location: • City Hall, 200 W. Washington Street • 6th Floor, Submittal Box • Due date: The Statement of Qualifications are due on November 4, 2016 12:00 pm, Noon, Local Time
Submittal Requirements • Bonding Statement is required • In a separate sealed envelope include a statement of the firm’s bonding capacity from a Surety Company • (A- or better for the prior four quarters) along with your SOQ submittal. (This bond statement will not be counted towards the maximum number of pages).

GROUNDS FOR DISQUALIFICATION Receipt of submittal after the specified cut-off date and time Too few copies of the submittal Deposit of submittal in the wrong location Violating the “Contact with City Employees” policy contained in this RFQ
