  1. ATTENTION PARENTS!!! DOES YOUR CHILD HAVE HEALTH INSURANCE? • What:You may qualify for LOW-COST or FREE children’s health insurance through: Children’s Medicaid or the state Children’s Health Insurance Program(CHIP). You may also qualify for the CHIP Perinate Program. • Why: • Children who do not feel well do not go to school ready to learn or to succeed. • More than 1 million children in Texas are uninsured. Nearly 80% of these children have at least one parent that works full time. • A family of four can earn up to $ 44,100 ($3,675/month) and still qualify! • Benefits: • Regular checkups, doctor’s visits, shots/immunizations, prescriptions, hospitalizations, labs, x-rays, care by mental health and medical specialists, hospice, vision and dental, basic prenatal care • INSURE ALL YOUR CHILDREN FOR $50 OR LESS FOR 12 MONTHS! • What Should I Include with My Application? • Include your child’s social security number and COPIES of the following: Child(ren)’s birth certificate for whom you are applying Proof of citizenship or permanent residency Copy of a recent pay stub OR income tax form and child support if applicable For cash income: a dated, signed letter from your employer may be used. Letters must include employee name, amount earned, length of employment, and a contact phone number for the company.
