Internship Process • Take the Peer Mentor 101 training • Participate in the peer mentoring teleconference each month • Prepare your portfolio • Gather all information that goes in the portfolio • Put it all in a binder or folder • Type up the information on the portfolio form • Clearly identify the skills you will offer to mentor others • Make is look professional so that you can sell your peer mentor services • CMH will identify the Supervisor to oversee the internship • Work with the Supervisor to make sure each peer mentor receives all required trainings from the CMH • Supervisor will work with the supports coordinators in the county to identify those that need peer mentoring • Supports coordinator submits an addendum to the PCP • New Goal • Goal to be met through peer mentoring • Create a process to match peer mentors with mentees
Internship Process • Clarify the billing process (paperwork and code) for CMH payment with the CMH Supervisor • Once a peer mentor and mentee are ready to start the internship process, the Supervisor will contact Tracy Vincent at the DD Council so that the documentation process for the internship can begin. • The peer mentor and the mentee will work out a schedule to spend at least 6 weeks, 2 to 3 half-days per week, developing the mentoring relationship • The person that will receive peer mentor services will fill out the form: My Individual Working Plan • The peer mentor will fill out the “per encounter” document each time they meet (Document name: Final Encounter Document) • At the end of the internship, the mentee evaluates the mentor with the document: Peer Mentor Performance Evaluation • At the end of the internship, the document: Peer Mentor Internship Performance Evaluation is filled out by the CMH Supervisor. A decision is then made, based on performance, whether the internship is complete or whether more work needs to be done to improve skills • Supervisor will send Tracy the required documentation after the internship has been completed • Tracy will set up an exit interview with the Supervisor, Peer mentor and when possible, the person who received the peer mentor services • Certification

Work is frequently unacceptable. 2=Below Standards: Opportunities for improvement exist however intern has not demonstrated adequate response to feedback. This rating represents good, solid performance. 4=Exceeds Standards: Frequently carries out tasks and activities that surpass requirements. Performance is the best that could be expected from any peer mentor.
Medicaid Code • When the goals of community inclusion and participation, independence and productivity are indicated in a person centered plan, a peer mentor’s unique skills may be utilized at a CMH as part of community living supports. • Medicaid Manual – Mental Health & Substance Abuse Section 17.3.H. PEER-DELIVERED OR -OPERATED SUPPORT SERVICES • Peer-delivered or peer-operated support services are programs and services that provide • individuals with opportunities to learn and share coping skills and strategies, move into • more active assistance and away from passive roles, and to build and/or enhance • self-esteem and self-confidence. • H0046
Building a positive peer mentor/professional relationship • What Professionals can do: • Identify opportunities to involve a peer mentor • Accept peer mentors as a partner • Help identify peer mentor’s strengths • Demonstrate/model problem solving skills • Demonstrate/model communication skills • What Peer Mentors can do: • Be honest with the individual you are working with • Commit to work to find solutions to problems • Follow through with promises of help • Assist the individual with setting and reaching goals • Help the individual build a circle of support and know the unique role of the peer mentor on the support circle • Direct individuals to the appropriate community resources • Know how and when to refer someone to another mentor or to a professional • Adhere to the guidelines and code of ethics established by the agency
