  1. REPORTING INJURIES You are required to: Report all injuries. Make sure your supervisor is notified.

  2. REPORTING INJURIES(CONTINUED) • APPENDIX I REPORT (2-part NCR form, this form is available in your department or school office. The report is completed by the victim or supervisor. A copy is sent to the EH&S Office and Human Resources Office. ) • Record of employee injury to be used for injuries which are so slight it is not necessary to seek professional medical treatment.

  3. Reporting injuries continued CS-13 REPORT (This form can only be completed by an officer from University Police or at the EH&S Office – Employees must call the 1-888-800-0029 number if professional medical treatment is being sought, e.g. Urgent Care or Physician ) To be used when professional medical treatment is needed and/or lost time from work.
