PORTFOLIO COVER The cover of your portfolio should show the title of your project and your name. You aren’t required to include a picture of yourself.
TITLE PAGE Alicia put all of her pages in plastic sleeves.
DIVIDER PAGE Alicia designed and printed divider pages. Divider pages aren’t required but, they add a nice touch.
PROPOSAL LETTER Here’s the version of Alicia’s letter that meets standard.
PROPOSAL LETTER RUBRIC Include the proposal letter rubric. It shows that you met standard on the letter.
PARENT/ GUARDIAN ACKNOWLEDG-MENT FORM Your parent(s) or guardian needs to sign this form. You can get one from our CP website.
LEARNING LOG You can get this form from the CP website. Download and type in the cells or print it and write in the cells. Use as many pages as necessary to record your 20 hours of project activities.
REFLECTIVE RESEARCH PAPER This research paper has been scored and it meets standard. Your paper has to meet standard before you can give your presentation.
RESEARCH PAPER RUBRIC The reflective paper rubric is the last required item of the portfolio.
DIVIDER PAGE If you decide to add more items to your portfolio, they will go in this section. Additional items aren’t required.
In this section of the portfolio you can add items such as: • Photographs • Receipts • Tickets • Business letters • Business Cards • Flyers or handouts that you prepared • Anything that provides evidence of you working on the project can go in this section.
