  1. This powerpoint shows the culminating project documents and the order in which they appear in a portfolio. • A checklist showing the required documents and the order in which they should appear can be found on Shadle’s CP web page at http://www.spokaneshcools.org/shadle--click on students--click on Culminating Project—click on portfolio.

  2. PORTFOLIO COVER The cover of your portfolio should show the title of your project and your name. You aren’t required to include a picture of yourself.

  3. TITLE PAGE Alicia put all of her pages in plastic sleeves.

  4. TABLE OF CONTENTS

  5. DIVIDER PAGE Alicia designed and printed divider pages. Divider pages aren’t required but, they add a nice touch.

  6. PROPOSAL LETTER Here’s the version of Alicia’s letter that meets standard.

  7. PROPOSAL LETTER RUBRIC Include the proposal letter rubric. It shows that you met standard on the letter.

  8. ANOTHER DIVIDER PAGE

  9. PARENT/ GUARDIAN ACKNOWLEDG-MENT FORM Your parent(s) or guardian needs to sign this form. You can get one from our CP website.

  10. DIVIDER PAGE

  11. LEARNING LOG You can get this form from the CP website. Download and type in the cells or print it and write in the cells. Use as many pages as necessary to record your 20 hours of project activities.

  12. DIVIDER PAGE

  13. REFLECTIVE RESEARCH PAPER This research paper has been scored and it meets standard. Your paper has to meet standard before you can give your presentation.

  14. RESEARCH PAPER RUBRIC The reflective paper rubric is the last required item of the portfolio.

  15. DIVIDER PAGE If you decide to add more items to your portfolio, they will go in this section. Additional items aren’t required.

  16. In this section of the portfolio you can add items such as: • Photographs • Receipts • Tickets • Business letters • Business Cards • Flyers or handouts that you prepared • Anything that provides evidence of you working on the project can go in this section.

  17. You can pick up copies of these documents from the library: • Your proposal letter • Your proposal letter rubric • Your reflective research paper • Your reflective research paper

  18. You’ll need to go to Shadle’s culminating project website these forms: • Parent Guardian Acknowledgment form • Learning Log form • http://www.spokaneschools.org/shadle--click on students--click on Culminating Project. • This and other portfolios are on display in the library.
