Materials Inventory Process • Retain original shipping boxes. • Verify boxes against the School Box Range Sheet. • District boxes are white; school boxes are brown. • Record ANY missing materials.Note on the security checklist. • Immediately notify DRC of any discrepancies. • Review packing lists and verify security ranges against the security checklist. Additional Materials • Consider whether materials can be reused before ordering additional materials. • Materials such as Oral Administration Scripts, CD-ROMs, Braille CDs, and sign language DVDs may be reused. • Use the Intra-District Transfer Form to transfer materials between schools in your district. • Carefully assess your need for additional materials. This form may be completed and e-mailed to the SC Project Team. • Orders received before 4:00 p.m. Eastern time will be processed on the same day and shipped via overnight delivery. DTCS p. 2

Form A Test Materials • Test Booklet • Oral Administration Script or Audio CD-ROM • ELA and Mathematics • Students must answer in their answer document (unless their IEPs state otherwise). • Use with any Form A ELA or mathematics test booklets when oral administration is specified in a student’s IEP. • Use when the testing of an LEP student requires an oral administration of the mathematics test. • Audio CD-ROMs are PC-compatible and are designed for use with a computer only.

Pre-printed Answer Documents • If pre-printed student information is incorrect, use an Override Linking Label and code the correct information by hand. • Coding the student’s name by hand onto a pre-printed answer document is not mandatory, but is strongly recommended. • Precoded and hand-coded information appear in different columns on the data file. • DRC uses hand-coded information as a double check if answer document information needs to be researched. New Student Precode Labels • NewStudent Precode Labels are produced for new students appearing on the March update precode file. TAM Appendix A
Labeling – Answer Documents New Student Precode Labels Use both labels for students taking both tests. Use one label – ELA or mathematics – for students taking only one test. Destroy the unused label in the pair. TAM Appendix A
Labeling – Answer Documents Override Linking Labels • Override Linking Labels mustbe used for students who did not receive a pre-printed answer document, a New Student Precode Label, or whose pre-printed information is incorrect. • Each sheet contains five pairsof white Override Linking Labels,side-by-side. Complete all information on each label. • Hand-coding the student’s name on the answer document is mandatory. If the student’s name is not hand-coded, the student will be reported as “No Name Available.” • Grid all applicable demographic information on the student’s answer document. TAM Appendix A
