Questions in main SHS In the last 12 months have you done any of these activities? • Dance • Played a musical instrument or written music • Rehearsed, performed or sang in front of audience, e.g. play or choir (not karaoke) • Painting, drawing, printmaking or sculpture • Photography as an artistic activity (not family or holiday 'snaps') • Made films or videos as an artistic activity (not family or holidays) • Used a computer to create original artworks or animation • Crafts such as textile, wood, pottery, etc. • Read for pleasure (not newspapers, magazines or comics) • Written any stories, books, plays or poetry • Other cultural activity How often in the last 12 months have you done this? • At least once a week • Less often than once a week but at least once a month • Less often than once a month but at least 3-4 times a year • Twice in the last 12 months • Once in the last 12 months • Don't know

Examples of C&S questions in module Participation / frequency - similar to questions in main SHS but with more detailed categories • Creative and cultural activities (27 categories) • Attendance at cultural events (21 categories) • Archive centres and record offices • Libraries • Historical sites • Museums and galleries • Sport (52 categories) – asked for 4 week and 12 month periods Example: In the last 12 months have you been to a library in Scotland? • Yes / No How often in the last 12 months have you been to a library in Scotland? • At least once a week • Less often than once a week but at least once a month • Less often than once a month but at least 3-4 times a year • Twice a year • Once a year

(showcard) • What would encourage you most? (showcard) Example (barriers): You said that you had not been to an archive centre in the last 12 months. • What were your reasons for not going? [Gym, Pitch, Swimming Pool] • Have you used it? • How do you rate quality of sports facility? • How do you rate quality of changing facilities? • Questions on live sports events attended • How many events attended in the last 4 weeks? • Which sport seen most often? • Did you have to pay to get in? • How many were for school events?

Examples of C&S questions in module Internet use In the last 12 months, have you used the internet, to look at any of the following websites? • Museum/gallery websites • Arts website • Library websites • Historical or heritage websites • Theatre/concert websites • Archive/record office websites • Sport websites • None of these Example (follow up): • You said that you had been on a theatre/concert website. (Y/N) Was this connected to any of the areas on this card? • Art, music, singing or drama group, etc. • Museums • Art galleries • Heritage (e.g. conservation, restoration, archaeological, helping out at Historical Society, etc.) • Libraries • Archives • Sport (e.g. coaching, refereeing, officiating, etc.) • None of these How much time have you spent on voluntary work connected with these areas in the last 4 weeks? On how many days in the last 4 weeks did you do this voluntary work?
