Laboratory phase:modification of interim metal cylinders • Guide pin 이나 laboratory waxing screw 위에 interim metal cylinder 를 위치시킨 후 안정적인 석고 base 를 제작하기 위해치과용 석고를 아날로그의 neck 까지 붓는다.
Repositioning index제작 • Stone base 에 low-speed 로 3 개의 notch 제작 metal cylinder 의 wall 주위로 baseplate wax 를 적용
Polyvinylsiloxane duplicating material 을 부어 mold 를 만든다.이 mold 를 이용하여 modified interim abutment 를 제작한다.
새로운 metal cylinder를 stone base 의 임플란트 아날로그에 연결하고 광중합용 복합레진을 silicone mold 의 빈공간에 채운 후 적층 중합시킨다.
각기 다른 platform 의 임플란트마다 위의 방법으로 interim abutment 를 제작한다.
Clinical phase:chairside use of the modified interim abutment for the fabrication off a provisional restoration • 이차 수술 후 healing abutment 를 제거하고 제작된 interim abutment 를 연결하고 방사선 사진으로 적합도를 확인
구강내에서 interim abutment 의 supragingival portion을 prep. 이 abutment 를 구강내에서 제거한 후 subgingival form 을 조정하여 적절한 emergency profile 이 나오도록 contouring 한다. 다시 구강 내에 연결한다.
적절한 크기의 Denture tooth 나 polycarbonate crown 을 선택하여 적당히 trimming 한 후 아크릴릭 레진으로 relining. trim,polish,stain 하여 인접치와 조화롭게 한다. 임시접착한다.
이 과정에서 수집된 정보를 사진이나 인상등을 통해 기공사에게 전달하여 심미적인 최종보철물이 나오도록 한다.
이 술식의 장점 • 따로 인상을 뜰 필요가 없다. • 심미적이다. • 이차 수술 후 바로 임시 치아를 해줘야 하는 경우 유용 • 연조직과 esthetic zone 에서의 emergence profile 을 control 할 수 있다. • 기공사가 심미적인 최종 보철물을 제작 할 수 있도록 정보를 제공.
