  3. Application Form • Stand alone document at the front of the new ACCR.

  4. Responsibilities of the Delegate. • Ensure the information on the ACCR is correct and complete • Ensure you complete the Approval section correctly (i.e. tick the correct boxes). • Do NOT back date your approval beyond the END of Assessment Date to cover EMERGENCY CARE. • If you are not approving emergency care, ensure that the EMERGENCY CARE section on Page 7 does not have a date or ticked box.
