7 nurses across the Outer East 3 KDGP & 4 ERGPA EFT of 3.7 – 1.5 KDGP & 2.5 ERGPA 41 sessions per week Sessions are offered at GP Clinics, Community Health, Local Government & Division Allied Health Service 25 at GP Clinics 6 at Community Health 1 at Local Government 9 at Division Allied Health Services Mental Health Nurse Service
An intake worker at each Division who receives & processes referrals Nurses are contacted via SMS or phone : New pt clinic X, Dr x, Dx, Severity (low, mod, high), frequency weekly/fortnightly/monthly Allows triaging of patients by nurses, scheduling appointments & creating patient file (electronic & paper based) Patient Referrals
Medicare Requirements • Intake worker receives claim forms weekly • Processed by intake worker and cross checked by Medicare approved Division rep. • Statistics collected based on patient demographics & session details • Tracking process to ensure all claim forms have been submitted by nurses and sent to Medicare
Supporting the Team • Fortnightly business and clinical meetings at each Division • Monthly joint Division meetings – clinical and business Clinical Meeting Business Meeting • Program Implementation • Accreditation (ISO / QIC) • Policies and procedures • Problem solving • Data analysis • Peer support • Patient triage • Debriefing • Education • External supervision provided fortnightly

Week 1: 3 Mental Health Nurses fulltime at the relief centers Week 2-4: 2 Mental Health Nurses fulltime at the relief centers Recovery phase: 1 Mental Health Nurse currently at the recovery center 2 afternoons per week ATAPS bushfire recovery funding will allow 5.5FTE mental health professionals to be put into the area. Funding guaranteed until January 2010. Bushfires and Beyond
