  5. The challengesDementia – A long term condition. • A long term condition • Much of LTC management is core work for primary care • What bits can primary care do? - GP core contract ? - or other professional with special interest ? • What should be commissioned from providers?

  6. A challenge to us all • Do we have agreed standards? • A pathway – who develops and owns? • Partnership v Commissioner/Provider split • Provider – responsive or proactive? • What is the USP of a provider service?

  7. Memory assessment – proposed standards • Early ? • Which patients ? • Exclusion of physical cause – role of scans? • Diagnosis includes subtype – how important? • Shared definition of MCI and actions? • Giving the diagnosis ? • Care planning? • The person - the carer ? • Education

  8. Living well with dementia& problem management • Long term support / navigation • Assessment and management of symptoms • Medicines management • Planning

  9. Discussion - the future • What is your USP? • Memory assessment • Longer term support • What needs to be in place? - agreed standards? /commissioning pack? - what else • What behaviours need to change?
