  2. Checklist Implementation • Pre CAS - 20 (50%) of 40 hospitals had already implemented the checklist • 55% <6 mos • 25% 6mos-1yr • 15% >1year • Post CAS - 8 (67%) of the 12 hospitals had implemented the checklist • 3 of the 8 hospitals had implemented the checklist during the CAS

  3. Checklist – Pre and Post • Pre CAS - Checklist used in 62% of all surgical cases • Post CAS – 83.5% • Checklist adapted to meet local needs in 50% of specific types of surgery • 23 (53%) of the 43 teams reporting indicated they had a method for identifying and recording “Good Catches”

  4. Examples of Close Calls • Identified equipment needed for case • Allergic to abx prior to administration • Wrong site surgery and wrong patient averted • Almand Oil and Na Citrate in same bottle stored in same fridge – potential error averted and storage changed

  5. Benefit of CAS • Raised the awareness and facilitated our moving forward with the checklist. • Excellent to hear from other institutions, speakers and presentations. The community of practice is very useful. • A rocky road to start, but I do believe we are well on our way now. We are starting to hear positive feedback about the teamwork - it is encouraging.
