  1. Notification Under the Academic Labs Rule • An eligible academic entity must notify its authorized state (or regional EPA office) that it is opting into Subpart K • Use the Site ID form (Form 8700-12) to notify • Site ID Form has been modified to include new box for Subpart K • An eligible academic entity must submit a separate Site ID Form for each EPA ID Number (or site) that is opting into the Labs Rule • When opting in, all laboratories operating under a single EPA ID Number (or at the same site) must opt in • Not all your sites have to opt in – but it is in your best interest to have all the laboratories on a campus operating under the same system

  2. If site has no EPA ID number, when it opts into Labs Rule, check: “Provide Initial Notification” 

  3. If site has EPA ID number when it opts into or withdraws from Labs Rule, check: “Provide Subsequent Notification”

  4. Existing Boxes A-C in Field 10 Use these boxes as before

  5. If an eligible academic entity is opting into Subpart K, then they should check box one and indicate what type of entity they are. More than one can apply.    NEW Box D in Field 10

  6. If an eligible academic entity is withdrawing from Subpart K, then they should check box two. NEW Box D in Field 10
