ALL MEDICATION must be administered from the clinic. • Prescription medication AND/OR over the counter medication must be brought to the clinic and be accompanied by a signed form by parent (and physician if needed for longer than 15 days) including the reason for medication, the dose, time and amount. These forms can be found at http://www.katyisd.org/dept/healthservices/Pages/Policy.aspx • Parents are asked to deliver all medication to the clinic (if at all possible). If the medication is a controlled substance, students MAY NOT transport medication to and from school. • Please call the nurse if your child is bringing medication, so I may ensure it’s safe arrival to the clinic. • All prescription medication must be in the original pharmacy container with the child’s name, medication date and dosing instructions. (Parents may request a 2nd bottle from the pharmacy so that the nurse can have the appropriately labeled bottle in the clinic.) • Medications to be administered 3 times per day, or every 8 hours are to be given at home.
