Quality Plan • Develop a Quality Plan that includes discussion on: • Increasing community tenure • Ensuring appropriate access to community based crisis intervention services 4
Quality Plan (cont’d) • Ensuring that all applicable OMS questions are answered fully • Ensuring optimum consumer engagement in outpatient treatment 5
Value Intensive Program • Engage program eligible consumers in effective and appropriate Outpatient Services • VIP consumers will be asked to select one OMS Provider to be the lead in coordinating their care 6
Value Intensive Program (cont’d) • Provider offers case management services, or • Makes case management services available through another provider organization 7
Foundation Phase • Develop and test Quality Plan • Use of clinic Inpatient census report • Use of Emergency Room Visit report • Adjust office practice to affect results 8
What’s Next? • Financial targets still being developed • Details of incentive payment discussions ongoing • January 2013 to June 2013; then based on fiscal year • Program pending DHMH approval 9
