The Community Connections Team of Huron County Board of DD presents….
Our Mission: All individuals served at the Huron County Board of DD will have a well-defined, person-centered ISP naming: interests, likes, dislikes, level of support requested, connections with like-minded disabled or non-disabled peers and friends, employment, self-determination of employment with an increased wage, increases of support within the community and paid supporters faded.
Got Transportation?? Ever miss a doctor appointment due to no transportation? Ever have a problem finding a wheelchair accessible transportation for a family member. Are you tired of spending half of your paycheck on public transportation to run your errands? Ever want to go to a movie late at night but have no ride? Are you tired of not finding an open handicap parking spot? Have you ever had a sick child at 3am and no way to get them to the hospital? Goals from the meeting • Ultimately we wanted a round-table discussion for all members in the community to voice their needs, wants, ideas and hopes in regards to transportation in our area • To see a collaboration between those supplying transportation and consolidate efforts to provide services to the greatest number of individuals • To identify that people have a need to be integrated into their community in order to make meaningful connections

What we take with us Jackie Tammy Jen Jaimie Connie Holly I’ve learned to focus on building relationships within the community rather than just getting out in the community I’ve learned that the services I provide shouldn’t be about quantity but quality! Everyone has something about them that shines. I’m determined to find that something and make it shine even brighter! Everyone has friends. We just help connect them and create opportunities! Not connecting is not an option! We need to get out of the way of ourselves and let individuals lead us!
