CR0420 – Multidisciplinary Team Discussion Indicator & CR0430 -MDT Date CR0420 Multidisciplinary Team Discussion Indicator is taken from the ‘Was patient discussed at MDT field’ in the Care Plan/MDT>MDT tab. CR0430 Multidisciplinary Team Discussion Date (Cancer) is the date recorded in the ‘Meeting Site/Date’ field in the Care Plan/MDT>MDT tab The COSD logic in SCR is these data only get reported to COSD/NLCA if there is an accompanying care plan recorded i.e. there needs to be a Care Plan Agreed Date even though this date is not reported to COSD/NLCA. See next slide for which fields need to be completed in order for CR0420 and CR0430 to be reported. If there is more than one Care Plan/MDT recorded, the data pulls from the earliest dated Care Plan recorded.
CR0460 – Cancer Care Plan Intent & CR0510 – Performance Status (Adult) CR0460 Cancer Care Plan Intent is recorded in the Cancer Care Plan Intent field on the Care Plan/MDT>Plan tab. CR0510 – Performance Status (Adult) is recorded in the Performance Status (WHO) field on the Care Plan/MDT>Plan tab. For these data items to report to COSD/NLCA, there needs to be a date recorded in the Care Plan Agreed Date field and at least the First Treatment recorded as well. If there is more than one Care Plan/MDT recorded, the data pulls from the earliest dated Care Plan recorded.
CR0490 – No Cancer Treatment Reason CR0490 – No Cancer Treatment Reason is recorded in the Not Treatment Reason field in the Care Plan/MDT>Plan tab. This field appears if ‘No active treatment’ is selected in the Cancer Care Plan Intent field. For this data item to report to COSD/NLCA, there needs to be a date recorded in the Care Plan Agreed Date field and at least the First Treatment recorded as well. If there is more than one Care Plan/MDT recorded, the data pulls from the earliest dated Care Plan recorded.
