Log-on to CDT User ID: Not case sensitive Password: Is case sensitive At least 8 characters with at least 1 lower and 1 upper case characters with 2 or more numbers.
Accessing a patient’s record: • No Consent =CANNOT ACCESS RECORD. • With Consent +Establish Relationship = • Can Access Record

Typical Error Messages:Search Timeout Typically you will get this message when your search is too large. Narrow the search before contacting your site coordinator.
Typical Error Messages:Session Timeout Communication problem Take note of any unsaved data, and click here to continue. When you get this dialog box – it typically means the session has timed out (more than 10 minutes of inactivity). Simple click logout and then login again.
Question 1 Can I access CDT from my home PC? No. Access to the CDT will only be from participating organization-owned computers or devices, and only from participant’s clinical location.
Question 2 Are my queries audited? Yes, CFRHIO will monitor access.
