A password box appears to allow opening as a read only file Opening as read only allows you to peruse the project but not make any changes to it. Enter the appropriate password To open for modification
Selecting New opens the welcome screen gives you Information about the process Click here when you are ready to start Clicking on the process step
Information for the project is entered here Each Project Can Contain • Company Name • Address • City, St & ZIP • Facility being assessed • Project Mgr • Phone / FAX • Email Information can be modified later by clicking on the Project button of the status bar
After entering the project information you can define up to 3 fields to use for sorting
The status bar gives direction and feedback along the way The lighted button shows the next step in the process The red indicator bar shows what has been completed Click on “Select Machine” to edit previous data
Click The Machine button to enter information for a new machine
A dialog box advises the new machine RA number and the number of machines left on the current authorization
Once the session information has been entered, the session ID shows on the status bar
Production machinery have associated hazards during setup and maintenance in addition to the production mode. These hazards could be the same but more typically are different and should be assessed separately. When starting to assess a new machine a dialog box appears to choose assessing in the three modes or just a single assessment. Non-production machines like drill presses and disk sanders may not require more than a single assessment.

The Assessor’s name and Date Evaluated defaults to the Project Manager’s name and the project date but can be changed Machine information includes • Department # • Machine Description • Machine ID • ANSI Std Category • ANSI Std Sub Cat • Assessor’s Name • Date Evaluated
However the machine ID must be changed Clicking on the “Select a similar machine” button allows you to duplicate any machine already completed

When choosing a 3 mode assessment, identification (-Prod, -Setup, -MTCE, -1, -2, -3) is automatically added to the end of the entered machine description and the machine ID
When copying from a similar machine you can also copy all the hazards, solutions and risk evaluations from the similar machine. For a 3 mode assessment these are copied to the production assessment
The 3 mode assessment shows up as 3 separate machines with the same information but with the identifier on the end of the description and ID Hazards, solutions and risk can now be copied from another RA through the main menu or entered individually via the status bar
A risk assessment is conducted to determine the level of risk before any safeguarding, and with the current safeguarding implemented. A risk assessment is conducted again once the appropriate safeguarding is installed. The design, construction, performance and implementation of the safeguarding are done in accordance with ANSI B11.19. The requirement and type of safeguarding is specific with each ANSI standard. Therefore by selecting the appropriate standard and sub category for the machine being assessed ONLY those potential hazards addressed in the standard are presented for consideration in the hazard identification portion of the software. This saves considerable time by not having to look through long lists of potential hazards that don’t apply to your machine.
Clicking on “Change Information” brings up the Session info screen with information from the last Session entered After the Session information is complete you can add more Sessions by clicking the Machine button, or evaluate the current Session for hazards
Hazardous situations are grouped as defined by ANSI B11-19 and individual machine standards The current Session is identified by Description & ID When entering Hazardous situations the dialog box appears to allow selection from a list of potential hazards
A short description defines the hazard group Only those groups listed in the individual machine standards are presented for consideration You are given the opportunity to skip over a group that does not apply by clicking “Next Group”
The “Previous Group” button allows you to go back to previous groups to check off missed hazards
Only those groups listed in the individual machine standards are presented for consideration If the specific group doesn’t apply to your machine or task then answering “Next Group” skips the hazard statements for that group
Once all of the presented Hazards have been selected, Click the “Review Selections” button
All selected hazards are then presented for review Check off any that you want to modify
Hazards selected for modification, are displayed one at a time to allow changes to fit the specific situation
If you decide a hazard statement is not needed it can be deleted here
If there are other hazards not included in the lists they can be entered here You can also enter applicable comments on existing guarding here
Modifications made are reviewed with other selections When you are satisfied with the hazard statements click “Done”
Risk is evaluated based on the recommendations of ANSI B11.TR3 Risk is evaluated first without any safeguarding
Subcategories are given severity ratings – a number relative to the degree of possible injury Severity of Harm is divided into the 4 major categories of B11.TR3
The rating is weighted based on the relative affect on the probability of occurrence Each category is given a rating reflecting the conditions observed
Ratings default to midrange and are changed from a dropdown dialog box Reliability of safety functions is further divided into 5 methods of control

Once an assessment is made of each area, the ratings are totaled to determine the category of probability of occurrence
