Southwest Continuum of Care Regional Information • SWCoC –spans over 19,100 square miles covering 23 counties. • SWCoC served over 6,183* individuals in calendar year 2012. • SWCoC is comprised of five domestic violence agencies; one active homeless shelter, one transitional housing program, and seven outreach offices for homeless prevention and re-housing assistance. • SWCoC has 2.5% housing availability, and eight of the 23 counties have 0% availability. • As a Continuum we are able to connect individuals with services beyond a bed to sleep in. We connect them with food, clothing, transportation, rental and utility assistance, job skills training, certification process, educational opportunities as well as case management and self sufficiency programs. • *excludes numbers from SAFE Center and Crossroads
