ACCIÓN SOCIAL Y SU PROGRAMA FAMILIAS EN ACCIÓN INVITAN A LA COMUNIDAD DEL NIVEL UNO DEL SISBEN O DESPLAZADA, QUE NO SE HAN INSCRITO EN EL PROGRAMA FAMILIAS EN ACCION Y TIENEN MENORES DE 18 AÑOS, QUE SE ENCUENTRAN EN EL MUNICIPIO DE SAN ANDRÈS DE CUERQUIA, PARA QUE LOS DIAS 06, 07 Y 08 DE MARZO DE 2009, SE PRESENTEN A INSCRIBIRSE EN EL COLISEO MUNICIPAL DE 8:00 AM A 4:00 PM. LAS FAMILIAS DEBEN VERIFICAR QUE SE ENCUENTREN INSCRITAS EN LA LISTA QUE FUE ENVIADA DESDE BOGOTÀ. PARA MAYOR INFORMACIÒN COMUNICARSE AL TELEFONO 861 81 73 - 861 82 06

  2. Para la inscripción se requieren los siguientes documentos:

  3. PARA LAS MADRES TITULARES Cédula de ciudadanía original o contraseña original y fotocopia legible Si es madre menor de edad, la tarjeta de identidad y fotocopia legible Se admite cedula de extranjería, pero no de extranjeros.

  4. PARA LOS MENORES BENEFICIARIOS Registro civil (RC) de nacimiento con parentesco (original o copia legible). El RC debe tener NIP o NUIP ( de 10 u 11 dígitos) y si lo tiene alfanumérico este solo puede tener como letras las 3 primeras.

  5. 2. Si es mayor de 7 años además del registro civil debe anexar tarjeta de identidad (si la tiene), pero solo en el caso de que el menor figure en la ficha del SISBEN de la titular. Para confirmar el dato se debe pedir el mismo al administrador del SISBEN del Municipio

  6. 3. Si es mayor de 7 años y estudia de segundo de primaria en adelante, certificado escolar del año en curso. Allí deben figurar los nombres completos del estudiante, el grado escolar cursado, nombre del colegio y el código DANE de la institución educativa, el cual contiene 12 o 14 dígitos.
