The Institute of Medicine in its 2005 report: Lost in Transition: From Cancer Patient to Cancer Survivor The Four Components of Survivorship Care recommended include: Prevention and Detection of New Cancers Surveillance for cancer recurrence or new cancers and interventions for consequences of cancer and its treatment.

  3. The 2005 Institute of Medicine Report 3. Psychological Distress Management 4. Coordination between Specialists and Primary Care Providers to Ensure that Survivors’ Health Care Needs are Met To address these components of care it was recommended that Cancer Care Specialists provide patients, care givers and PCPs with a Survivorship Care Plan.

  4. SURVIVORSHIP CARE PLANS What are they? When Should SCPs be Provided? To Whom Should SCPs be Given? What Should be Included in the SCP? Do you or your institution have standard SCPs or do you use an online source: such as ASCO’s Cancer.net or Journey Forward or ACS Guidelines?

  5. Management of Premature Menopause in Breast Cancer Survivors <50 years of age How common does this occur after chemotherapy or hormonal therapy? Is Premature menopause always a Bad Thing? What can be safely & effectively done to ameliorate symptoms?

  6. Nutrition, Weight Control and Fatigue Foods to Eat? Foods to Avoid? What about Vitamins? Is alcohol a No-No? Does achieving and maintaining a BMI well under 30 really matter? How important is regular physical exercise?

  7. Anxiety, Depression & Fear of Recurrence Allaying Fear without False Hope? The Role of Support Groups -Do they help? -Are they beneficial for everyone? When to Call In the Psych Team Chemo Brain – The gift that keeps on giving
