Integration • Better Care Fund (BCF) • Pooled budget between health and social care • In 2015/16 - £24M • Working closely with all providers to develop a shared vision for Integrated care in Ealing – including Voluntary Sector (co-design) • Co-commission services, working closely with the Health and Wellbeing Board • Aligned services to GP networks • Whole Systems Integrated Care (WSIC) – Pioneer Site • Shared expression of interest to deliver new model of care for (initially) the over-75s with one or more long term condition • Partners are: Healthwatch, ECN, Ealing ICO, West London Mental Health Trust, Ealing CCG, London Borough of Ealing
Whole Systems Integrated Care – our plans…. • Care Co-ordination: fundamental to successful delivery of an integrated system is the role of the care coordinators • Coordinate care around networks of practices • Support Care planning & Case Management • Provide proactive support for the most vulnerable • Deliver Improved Access and Outcomes • Deliver Improved communication and co-ordination across the system • Care Navigation: just as critical to successful integration • Provide support for people to navigate the health and social care system (statutory) • Navigate through non-statutory services such as advice services • Advocacy
Patient A’s journey: an example Patient A has type 1 diabetes, long-term complications including eyes, heart, nerves and feet problems, weight issues, and smokes • High risk – care co-ordinator appointed • Range of therapies in one place, including foot and dietetics • Outpatient appointments available for more specialised treatment • Education provided for self management • Support available from voluntary sector based in the Health and Wellbeing Centres • Urgent care available if needed
How the voluntary sector can support the CCG’s commissioning intentions • Self-management of long term conditions • Signposting and navigation • Prevention and early intervention • Early identification of needs and timely referral to services • Health promotion • Help us to manage demand • Add value to the Health and Wellbeing Centres • Advocacy for patients to ensure improved access • Help service users to have a voice and be involved • Cascading key health messages to the local population • Bidding to deliver services • Equality briefing for CCG clinical leads and staff • Community tour for CCG clinical leads • Promote volunteering in health settings Keeping people well and out of hospitals Transformation and integration of services Involving patients in the design and delivery of care Briefing and raising awareness of local needs
