Performance Delivery – overall summary (1/4) Supporting Measures Headline Measures • The achievement is based on the current months performance (although this may relate to a previous month due to the availability of data) • Where a table indicates progress, the progress relates to the most recent published data where available or monthly or weekly leading measures for each indicator. Additional Public Health Measures • Public health measures- change in indicator- breast screening uptake for 50-70 yrs is now being measured instead of coverage. 2010-11 and 2011-12 performance has been amended to reflect this. CVD Mortality and Cancer Mortality- no longer using a 3 year rolling average as actual but instead using the annual rate. • Note: Cluster have re-issued RAG thresholds which may impact on how some of the targets are reflected in Cluster documentation. The new thresholds will be used in the Board reports for 12/13 reporting. 3

  11. 2011/12 Performance Delivery - Summary (6)GP Consortia A summary of GP Consortia performance is noted below however please refer to appendix 2 for the more detailed performance dashboards by Consortia • Immunisations (Q1 and Q2): • Kingsbury met all 6 immunisation indicators. • Wembley and Harness have met the Age 2 Hib Menc indicator. • Willesden achieved the Age 1 DTaP indicator • Kilburn did not achieve any of the immunisation indicators • Smoking quits: • No localities have met their YTD Oct plan for smoking quitters • Breastfeeding 6-8 weeks (Q1 and Q2): • Harness, Kilburn and Wembley met the 95% coverage target. • Kilburn and Wembley met the prevalence target of 72.7%. • Breast screening coverage: • Kingsbury and Willesden achieved the 70% target for women aged 53-70 yrs screened for breast cancer in the last 3 years as at 31st March 2011. (based on provisional 2010-11 KC63 data) • Kingsbury and Wembley were above the PCT % of 28.5% of women aged 47-73 yrs invited for breast screening in the last 12 months as at May 2011 • Cervical Screening: • Coverage as at Q2 2011-12- all 5 clusters met their 2% above baseline incentive target for women aged 25-49 yrs. Only Kilburn and Willesden met their 4% above baseline incentive target. Wembley met the national target of 80% for women aged 50-64 yrs.

  12. 2011/12 Performance Delivery - Summary (7)Social Marketing/Health Promotion Breastfeeding • 8 Breast feeding Peer Supports in post and two awaiting HR clearance. • DVD produced for mothers on breastfeeding • 19th January 2012 - Breastfeeding event in Brent Town Hall promoting breastfeeding and other priority areas. NHS Health Checks • NHS Health Checks promoted in Brent magazine (Dec/Jan edition) • NHS Health Checks on Life Channel (Jan and Feb) through GP surgeries in Brent Cervical screening • January-February: A health promotion specialist in place to promote cervical screening in younger women Smoking, Immunisations, NHS Health Checks • Social marketing events covering all the above took place in December at Brent Health Partnership Day, Kilburn Square and at Freuchen Medical Centre Smoking Cessation • Stop Smoking team will be located at Asda Colindale, Sainsbury's Kenton, Law Medical Centre, Oxgate Gardens Surgery on dates throughout January

Breastfeeding 6-8 weeks • In Q1, 98.1% of infants due for a 6-8 wk check had a feeding status recorded against them and 75.2% were being breastfed at 6-8 wks. • In Q2, 95.5% of infants due for a 6-8 wk check had a feeding status recorded against them and 75.2% were being breastfed at 6-8 wks.
