Only supervisors can give FOSA evaluations in PATH. Current group = supervisor Select Begin New Performance Appraisal
Click Search. This will bring up the names of all your direct reports. Hint: Do not fill in any of the fields!
Select Start Action under the name of the employee you wish to evaluate
FOSA Evaluation Type a title for the Evaluation. Include the word FOSA in the title Select Continue to Next Page

Complete as necessary. Select Continue to Next Page (not shown)
Employee Relations Actions • Evaluation Inactive: Cancels Evaluation • Submit Final to Supervisor: No changes necessary. Evaluation is ready to be sent to employee. • Submit to Supervisor for Changes: Changes necessary. Supervisor makes changes and return to Employee Relations.
Send FOSA to Employee • Once Employee Relations submits the final FOSA Evaluation to the Supervisor, the Supervisor conducts face to face meeting with employee to discuss. • After the meeting, Supervisor sends FOSA Evaluation to the employee in PATH.
Employee can make comments if desired. Employee certifies receipt of FOSA. Employee selects Continue to Next Page
