Presentation to CA Environmental Health TrackingMarch 19, 2003 A project of the California Department of Health Services and Impact Assessment Inc.
Project Goals • Assess the need for biomonitoring within the State • Identify new laboratory biomonitoring methods or expand current methods (given the needs) • Identify public health research projects within which to achieve goals 1 and 2. • Write grant to implement plan. A project of the California Department of Health Services and Impact Assessment Inc.
Project Background • CDC funded 33 States to a conduct planning process for biomonitoring • Final Plan due June 30, 2003 • Distribution of funds: • $200,000-$3,000,000 • Larger funding to consortium grants • 2-10 grant awardees A project of the California Department of Health Services and Impact Assessment Inc.
Planning Structure • Needs Assessment of health hazards & health effects (surveys, inventory, reports) • Selection of Biomonitoring Projects • Implementation Plan with Partners A project of the California Department of Health Services and Impact Assessment Inc.

Researcher Survey Results • Analyze samples • Increased throughput • Create population reference ranges • Develop methods & reference support A project of the California Department of Health Services and Impact Assessment Inc.
Baseline Laboratory Inventory • Broad range of biomonitoring methods and chemical testing • Potential to form Laboratory Network with State, Local, and Private Partners • Laboratory has capability to test for Toxic Substance of concern • Primary laboratory challenge is capacity to run large numbers of samples to meet research need A project of the California Department of Health Services and Impact Assessment Inc.
Needs Assessment Conclusions • Community and Researcher survey respondents agree that the toxic substances of most concern are Pesticides, Heavy Metals and ETS. • This is supported by a review of selected reports. • Laboratory capacity either exists or could be developed by the Laboratory Network A project of the California Department of Health Services and Impact Assessment Inc.
Important areas for biomonitoring • Pesticides • Heavy metals (lead, mercury, arsenic) • Cost-effective laboratory methods • Emerging Concerns (PBDEs -brominated flame retardants, and Phthalates - plasticizers) • Exposure markers for air pollutants (benzene, PAHs) A project of the California Department of Health Services and Impact Assessment Inc.
Methodology Selection Criteria & Decision Flow Step 1: Scientific Criteria (toxicity, exposure, preliminary lab feasibility, hypothesis, study design, study population) Step 2: Collaborator Infrastructure Step 3: Responsible Research Step 4: Public Health Benefit, Support & Impact Step 5: Sampling & Laboratory Feasibility A project of the California Department of Health Services and Impact Assessment Inc.

Current Planning Steps • Meetings with Partners to understand each others needs and strengths • Define roles & timelines for each collaboration • Continue planning & conversations with collaborators • Develop tiered approach based on potential funding rewards A project of the California Department of Health Services and Impact Assessment Inc.
Opportunities: Implementation Plan • Target chemicals • Health Endpoints • Normative Data • Breast milk methods development A project of the California Department of Health Services and Impact Assessment Inc.
1. Target Chemicals • Pesticides (OPs, Pyrethroids, Organochlorines) • Noncoplanar PCBs • PBDEs • Metals (lead, mercury, metals panel) • Phthalates A project of the California Department of Health Services and Impact Assessment Inc.
1. Target Chemicals • Consistent with Pew Report recommendations for exposure tracking • Consistent with CA Biomonitoring Needs Assessment Report A project of the California Department of Health Services and Impact Assessment Inc.
2. Health Endpoints • CHARGE:What are the environmental and genetic factors that influence the risk and severity of autistic and developmental delay/ mental retardation? • CHAMACOS:Is in utero and/or postnatal pesticide exposure associated with poorer neurodevelopment or somatic growth? A project of the California Department of Health Services and Impact Assessment Inc.
3. Normative Data • A normative study uses samples from “normal” healthy people to provide “normal” background ranges of substances in the population. • Tremendous value for comparative purposes such as in determining whether levels of environmental chemicals are unusually high among certain individuals • Monitoring trends in the population A project of the California Department of Health Services and Impact Assessment Inc.

Normative Data – MS-AFP samples • Laboratory feasibility for maternal serum from the alpha-fetoprotein (MS-AFP) specimens • Exploratory study with GDB • Normative data for pregnant women • Sample size and contamination? A project of the California Department of Health Services and Impact Assessment Inc.
3. Normative Data – CHARGE Study • Use the 700 controls (and their parents) to establish reference ranges. • Provide laboratory support for the analysis of selected chemicals requested by the CHARGE study. • Perform additional analyses among the controls to determine levels for metals in urine.
