  2. INTRODUCTION Les complications concernant l’articulation fémoro patellaire sont fréquentes après une prothèse totale du genou les luxations de la rotule sont rares

  3. OBSERVATION • Homme 89 ans PTG Suites « difficiles » Luxation « ERECTA »

  4. OBSERVATION Colonne post + paroi post Transversale + paroi post post Section large AE réinsertion aileron interne par ancre

  5. OBSERVATION • Radiographie post opératoire: rotule en place

  6. DISCUSSION • Les complications d’origine fémoro patellaire après PTG sont une cause fréquente de reprise chirurgicale • Les subluxations sont fréquentes • Les luxations sont rare (0,4%) • La luxation « érecta » n’a pas été rapportée

DISCUSSION • Rechercher: • une instabilité rotulienne pré opératoire sur dysplasie • défaut de positionnement des pièces tibiales et fémorales • Notre cas : • Pas de défaut majeur de positionnement des pièces fémorale et tibiale • rotule épaisse (insuff de coupe) D’où encombrement sagittal impt

  8. CONCLUSION • Les complications de l’articulation fémoro patellaire après PTG sont fréquentes et sources de douleurs voire de luxation de la rotule • Erreurs techniques +++: • non respect de l’encombrement antéro postérieur • troubles rotatoires de l’implant tibial et/ou fémoral • déséquilibre des haubans une attention particulière à ces différents facteurs avant (et pendant!) la réalisation d’une PTG
